In New York, there is a lot different programs to get free home care service base on age and the benefit cards that you have. Let's try to summarized all information.

Everyone who is interested in applying home care should fall into one of the following categories.

1. Medicaid only

Need check the Medicaid ID for any restriction code,

if cleared then Qualify for Long Term home care,

however might affect Doctor depend on the plan that you choose

There are different plan like MCO, MLTC, MAP, PACE, OPWDD, Wavier NHTD & TBI

2. Medicare only

Qualify for Short Term home care,

but available mostly after Hospital or Nursing home

Can Contact to join RPM (Remote Patient Monitor) program to apply Blood Pressure Machine, Blood Glucose Meter and other medical supplies.

Both Medicaid and Medicare

Need check the Medicaid ID for any restriction code,

also Qualify for Short Term home care,

Mostly doesn't affect Doctor

Can contact me for information to qualify Medicaid,

Example if seniors income too high can do Pooled Trust,

if have house and assets can do other Trust.

Medicaid allow patients with Trusts to bypass the limit.
